Eligibility Requirements
- Any active MIH or CP program may nominate its payor partner(s).
- Self-nominations are welcome if supported by a collaborating provider organization.

Recognizing Excellence Across the Spectrum of Payor Innovation
Eligibility categories include:
For large or multi-state health plans demonstrating measurable improvements or scalable MIH innovations that influence national practice.
For Medicaid MCOs or regional health plans that have advanced MIH reimbursement, launched pilots, or strengthened regional payor–provider partnerships.
For community-based plans or hospital-affiliated payors driving local MIH success through partnerships, tailored programs, or targeted high-value interventions.
For ACOs, physician organizations, or clinically integrated networks that integrate MIH into value-based models, care coordination, or readmission reduction strategies.
